WHAT MUST I DO IF MY VISA IS LOST OR STOLEN?
If your passport with visa is lost or stolen, or the page of your passport with your U.S. nonimmigrant visa is missing, it is vitally important that you report the lost visa as soon as possible to protect you from identity theft or another person misusing your nonimmigrant visa.
Here are the steps to follow:
1. FILE A POLICE REPORT. If the loss/theft took place in India, you must report the loss of your passport and visa to the local Indian Police and obtain a police report. If this happened overseas, please obtain a police report from that locale.
2. EMAIL US at NIVND@state.gov

- Your name(Surname, Given Name(s))
- Date and place of birth
- Nationality
- City where your visa was issued (Mumbai, Chennai, New Delhi, Hyderabad, or Kolkata)
- Date of visa issuance, if available
- Nationality of your passport
- Number of your lost passport as well as the issuance and expiration dates, if available
- Your current address and phone number
- When and how your passport with visa was lost or stolen
ATTACH:
- A copy of the police report
- A photocopy of the lost visa, if available
- A photocopy of the bio-data page of your lost passport, if available.

WHAT MUST I DO IF MY VISA AND PASSPORT ARE DAMAGED?
Apply for a new passport and visa. Damaged travel documents may result in increased scrutiny by airline and immigration personnel, which may include denied boarding for your flight or onward travel. The U.S. Embassy and Consulates in India cannot vouch for acceptance of a damaged visa or passport by airline personnel or immigration officials. You will need to apply for a new passport and new U.S. visa.
